Appointments

We schedule appointments Monday through Friday, 8:00 a.m. to 5:30 p.m. Surgery and test scheduling can be done in our office.

Initial Visit

On your initial visit, please bring:

• Your insurance card
• Your referral, if required by your insurance company
• List of allergies
• List of current medications (prescription or over-the-counter) including name, dosage, frequency
• Related x-rays
• Records from the referring physician

Payment and Insurance

Payment is due at the time of service. For your convenience, we accept Visa, MasterCard, or debit card. If your visit is covered by your health insurance, we can file your insurance claim if you bring your plan's membership card.

We are participating providers for Blue Cross and Aetna. We also accept Medicaid and Medicare.

If you are covered under Worker's Compensation and they do not honor the charges, you are solely responsible for the account balance within thirty days.

Drug and Device Studies

We participate in many worthwhile studies of new drugs, new devices, and new indications. This benefits our patients by giving them access to new cancer agents, new therapies such as the oral pills for sexual dysfunction, and therapies that they might not afford.
